I don't appear to able to send mail to other users, I tried earlier but it wouldn't let me. B-(
Anyway a template is just like a page, so a "Donate" template would reside at the URL:
http://wiki.contribs.org/Template:Donate
Create it and copy 'n paste the wiki markup for what you have already done relating to Donations into it and save it. Then where you want to include the template on any other page just use <nowiki>{{Donate}}</nowiki>.
